Form

A form is a standardized report template an appraiser uses to present the results of an assignment. Standardized reporting helps lenders, underwriters and investors find consistent information across appraisal reports, although the specific format depends on the property, assignment and reporting requirements.

Residential appraisal forms have historically included the Fannie Mae 1004 for one-unit residential properties, the 1073 for individual condominium units, the 1025 for two- to four-unit properties, the 1007 for a single-unit rent schedule and the 1004D for updates and completion certifications.

The appropriate reporting format depends on the assignment and applicable requirements. The GSEs are transitioning residential appraisal reporting to a dynamic, data-driven format under UAD 3.6. Rather than relying solely on fixed form fields, the new format organizes appraisal data in a standardized dataset that can adapt to different property characteristics and reporting needs.
